It’s breast cancer awareness month and once again we are thinking pink. While wearing pink is a great visual reminder that this disease exists; breast cancer awareness is much more than a month and way more than rocking a ribbon. It’s a yearlong call to action; to ensure we here in Montserrat continuously highlight the causes and risk factors, commit to practicing prevention strategies such as healthy living and monthly breast self-examinations and push for improved access to mammograms and better treatment and care of persons’ living with breast cancer. Helping Hands for Glendon recognises the importance of having a hemodialysis machine on the island of Montserrat. We are currently waiting for Mr Atwater’s report to see if this is feasible. To have a dialysis machine on the island would be a game changer whereby providing assistance with medical procedures that will save lives, and giving people the opportunity to return home where they have not been able to. On day four of Mr Atwater’s visit to Montserrat he had an interview at ZJB Radio with Mr. Herman Sargeant - Station Manager. On day three of Mr Atwater’s visit to Montserrat with members of the Helping Hands for Glendon team, he visited Glendon Hospital to look at the facility and where a haemodialysis machine could be situated. The tour of the hospital went well and in our next few posts we will share the outcome of what happened.

On day two of Mr Atwater’s visit to Montserrat with members of the Helping Hands for Glendon team, he had a meeting with the Montserrat Utility Service- Water Division (MUL). They discussed the water supply in Montserrat and how it will impact having a haemodialysis service on the island. Mary Tuitt explains the role a dialysis nurse … A dialysis nurse plays a critical role in the care of patients with chronic kidney disease. These patients lives depend on a supportive treatment known as hemodialysis. The Dialysis nurses’ duty is all dependent on the kidney doctor’s (nephrologist) orders and a wide range of Clinical and supportive duties. These duties include: ◦ Prepare patients and equipment for dialysis. ◦ Access and manage ports such as grafts, fistula and catheter. ◦ Patient assessment, pre-, during and post-dialysis sessions. ◦ Monitor vital signs and recognising and addressing complications. ◦ Administer medications and adjust settings based on physician orders. ◦ Strict infection control practice to prevent infections related to dialysis Access port. ◦ Recognise and respond to dialysis-related issues such as sudden drops in blood pressure, bleeding or cardiac issues. A dialysis nurse must possess strong clinical skills, empathy and the ability to handle complex and sometimes stressful situations.

Tuesday 25th June 2024 - Sitting at the Legislative Assembly the Ministry of Health looks to the viability of hemodialysis dialysis services on the Island of Montserrat. Responding to a question posed in the Legislative Assembly as to the possibility of offering haemodialysis services on the island, The Honourable Charles Kirnon Minister of Health stated: ‘There are certain factors that need to be considered such as water supply treatment and waste disposal. The maintenance of the machine also has to consider other guidelines in ensuring that the unit facilitates infection and prevention issues particularly to prevent cross infection of blood bone disease and other infections to the user will also have to be adopted to ensure, the safe delivery of this type of service. The procurement of dialysis supplies also to be considered regionally...’ The Ministry of Health was approached by a charity organisation (Helping Hands for Glendon) which has facilitated an assessment of the visibility of having a haemodialysis service on the island. The Ministry of Health looks forward to the outcome of the piece of work and to evaluate whether it is suitable for the island of Montserrat. Did you know that based on a study carried out in Montserrat in May 2021 on the ‘Prevalence of chronic kidney disease (CKD) and associated risk factors on the island of Montserrat’ 355 participants took part in the study which showed that 36 under 40 year olds were at risk of CKD 42 - 40 to 49-year-olds were risk of CKD 65 - 50-59 year olds were risk of CKD 213 over 60 year olds were risk of CKD 213 females and 142 males were risk of CKD And 201 participants had hypertension The conclusion from the study found that primary interventions can help reduce the impact of chronic kidney disease for example making healthy lifestyle changes to ones diet and fitness routines. It’s important pay attention to the colour of your urine as it can provide valuable insights into your health. Dark amber, red, or brown urine could indicate that your kidneys are not functioning correctly. However, it’s important to keep in mind that darker yellow urine could simply mean that you need to drink more water to stay hydrated. I have been a registered nurse caring for the community for 11 years. I have seen many patients through their journey with renal failure. Renal failure is when your kidneys are damaged and cannot do the important jobs they are required to do. Healthy kidneys clean the blood, removing any harmful waste products and excess fluids and turning these into urine to be passed out of the body. Dialysis is a procedure that involves replacing some of these functions when your kidney no longer works. Having a renal dialysis machine in particular a haemodialysis would enable patients to visits a clinic/hospital 3 times per week lasting 4 hours. This would be very beneficial for the people who live on the island of Montserrat as it will enable patients to visit a place which is accessible to them without having to travel to another country for treatment. This may be very costly for the patient and impact them on their well-being. Subsequently, those who want to return home who have renal failure are not in a position to do so. How can people donate to Helping Hands for Glendon? There are several ways that persons can contribute to Helping Hands for Glendon. For one thing they can donate through the Charity’s account 652-1479 at Bank of Montserrat. Donation boxes are placed conveniently at some of the major supermarkets and stores in Montserrat, so that they are easily accessible by persons wishing to donate. How often do the management committee meet and what do you discuss? Though the management committee is geographically separated, regular discussions are held via online platforms. Formal quarterly meetings are scheduled, but the Committee may meet sooner as situations arise that need to be addressed. Discussions are held on a range of issues relating to the ongoing healthcare needs of Montserratians and the equipment needed to address these concerns. Consideration is given to creating partnerships with other organizations with a similar interest in meeting the healthcare needs of Montserratians. As well, discussions are held on ways of connecting with persons in the diaspora who are willing to join hands with HHFG in the ongoing efforts to improve health care in Montserrat. Transparency and accountability for funds received and equipment purchased by the Charity, are a standing agenda item.

How does Helping Hands for Glendon remain financially transparent? Working at MS Osborne Montserrat for nine and half years as a cashier and account clerk and also as an administrator for a day nursery in London for 43 years has helped me hone my financial skills to maintain accurate financial records. The Helping Hands Committee believe in upholding the principles of transparency by giving monthly updates on our social media platforms of what we have raised and spent. We also do our due diligence by ensuring that our accounts are monitored/inspected every financial year.

Four years on has the mission, vision and goal changed? I think our mission has remained consistent throughout the four years which is to raise funds for vital life-saving equipment for the community of Montserrat which we have achieved. To improve the health services and diagnostic capabilities of the Radiology Department of Glendon Hospital and Montserrat, the Ministry of Health procured an A64 slice CT scanner and 3-D mammography. While the availability of these advanced imaging equipment is important, training of the human resources who will operate these equipment is also paramount. Therefore as a precursor to events to the installation of these equipment, it was determined that refresher training would be necessary for both Radiographers employed by the Ministry of Health. Both Radiographers are trained in doing CT and Mammogram examinations, however, due to the absence of these modalities on the island they have not been able to have active practice and or interface with these modalities. A two-week attachment of hands-on and observership was secured by the Ministry of Health at the UHWI Radiology Department in Jamaica. From July 3rd - 14th 2023 Miss Antoinette Silcott, Senior Radiographer and Sonographer journeyed to Jamaica and joined to this department on the first attachment. Miss Silcott, was scheduled to work a one-week 8-4 shift in the CT department and another week in the Women’s Imaging Department with a focus on mammography. This attachment allowed observation and practice in: patient preparation and management, patient positioning, patient care, examination protocol set up/ planning, examination completion and dispatchment of images and patient. Ms Silcott was allowed to participate in some troubleshooting and preventive maintenance- calibration and quality assurance program routines that will be necessary with the introduction of these diagnostic tests on the island. Though the training was short and intense Ms Silcott expressed appreciation and anticipated participation in future training for the staff within the department. Appreciation and gratitude are expressed gratitude the Ministry of Health and Helping Hands for Glendon for providing funding and support. In June this year we focused on how the Vital Signs Monitor has had a positive impact at Glendon Hospital since being donated by Helping Hands for Glendon. Repost… I am Nurse Petreece Perkins an Enrolled Nursing Assistant who is based at the Casualty department of Glendon Hospital. My role is to carry out the necessary care for all patients. This includes monitoring all vital signs. Therefore, the Vital Signs Monitor donated by the Helping Hands for Glendon Team affords patients the opportunity to receive the best possible care. With this machine, we can identify any early issue with patients by assessing their oxygen levels, heart rate, temperature and blood pressure. Repost … Hello, My name is Jeanette Brade, a Registered Nurse /Registered Midwife and presently Home Care Manager of Margetson Memorial Home for the Elderly on a Montserrat . On December 20, 2021, in my capacity as District Nurse at St. John’s Health Centre, I was elated to accept a foetal monitor that was donated by Helping Hands for Glendon, one of the charity organizations in Montserrat. This valuable piece of equipment is a welcomed asset to our antenatal sevices. It enables the midwife to monitor the foetal heart during pregnancy and helps to monitor the overall general health of the baby. It therefore makes health care more accessible, acceptable and affordable to our mothers because they can have this service at the Health Centre instead of being referred to the Hospital. This service is much more convenient for the expectant mother, the midwife, and the Obstetrician/gynaecologist. Therefore, today, on behalf of all expectant mothers present and future as well as the medical and nursing fraternities I extend sincere gratitude to you Helping Hands for Glendon for the timely donation of this life changing equipment to support Midwifery and practice on Montserrat. This post acknowledges our 3rd anniversary which was in February 2023. We wanted to highlight some of the new initiatives we aimed to fulfil this year and we are proud to say that we are achieving them, thank you for your continued support this year. Three years on has Helping Hands for Glendon mission, vision and goals changed? No, our mission, vision and goals have not changed. Even though the Government of Montserrat 🇲🇸 has recently announced purchasing the island’s first CT and Mammogram scanners through funding from the Foreign and Commonwealth and Development Office ( FCDO) we are truly pleased with this initiative. However, one of our aims and objectives was to support training of healthcare workers and therefore we are willing to assist with the training of the technicians who will be the operators of these two major equipment. PSA test is a blood test, called a prostate-specific antigen (PSA) test, which measures the level of PSA and may help detect early prostate cancer.

Dear Friends of Helping Hands for Glendon, We are really proud to announce that we have raised to date through: Go Fund Me £6615 Bank of Montserrat $73,037.60 EC dollars PayPal £518.91 Cash Donation £931.30 Cash Donation $200 EC dollars With conversions, we have raised to date an amazing $100,859.57 EC dollars With what we have raised we are happy to announce we have purchased a: * Telescopic Colposcope (without pantographic arm), LED light source, F:300mm Which is going to used in hospital in the gynaecology clinic to support women’s health concerns. * BT350 Advanced LCO Foetal Monitor. This piece of equipment has been donated to St John’s Clinic for use within our primary healthcare setting. * Connex 6700 Series Vital Signs Monitor BP, SpO2 (Nellcor), Early Warnings. This is now being used in Montserrat. * Support training towards the new CT scan and mammogram machine - more information to follow. Helping Hands for Glendon would like to share Lauren Piper’s inspirational journey of surviving breast cancer. Click the link in our bio to watch. Since our charity was founded our main focus was to raise funds for secondary medical equipment and we are proud to announce through raising awareness of the health needs on the islands the Montserrat Government have announced that the Foreign Commonwealth and Development Office has given $9 million to Montserrat to purchase medical equipment e.g. for a Mammogram and CT scan and other much-needed resources. Therefore we have decided to move our focus to primary care (community and preventative) to continue to promote awareness around raising funds for vital life-saving equipment.
